The North Tempe Health Center is a nurse-managed health center supported by the ASU College of Nursing and Health Innovation. North Tempe Health Center aims to promote healthy lifestyles and behaviors, improve quality of life, and provide access to healthcare for uninsured or underinsured individuals, children and families in Tempe.

The Escalante Health Center provides wellness programs and primary care services for uninsured and underinsured individuals, children and their families of Tempe, in addition to community outreach at health fairs and schools. Staff at the Escalante Health Center work with social service agencies as well as students and faculty from various health disciplines to develop wellness programs, and to give students a interdisciplinary learning experience to health promotion in the community. The Center integrates community health services with faculty practice, education, and research.

Serving the public for more than three decades, Community Health Services (CHS) Clinic is a unique family healthcare clinic where nurse practitioners and nurses are the primary providers of care. Clinic services are focused on health promotion and are intended to foster and maintain wellness, and reduce health risks by providing health information and education, health maintenance, and the treatment of minor illness.

Breaking the Cycle Community Health Care is a unique nurse-managed clinic located in the heart of downtown Phoenix at Grace Lutheran Church. The clinic staff is dedicated to serving families who have nowhere else to turn for care. They are the people in our community whose income or circumstances prevent their access to health insurance.
