Care Partners Reaching Out – Long Term Care (Virtual) (CarePRO-LTC)

CarePRO-LTC Virtual addresses the ongoing needs of family and friends caring for individuals with dementia (Alzheimer’s disease and related dementias – ADRD) who have been placed in long-term care facilities.  It's skill-focused and support meetings occur over approximately 5 weeks. CarePRO LTC consists of 5 weekly group sessions held via Zoom, lasting 2.5 hours each intermingled with 5 weekly individual telephone coach sessions lasting approximately 30 minutes each. The program’s small interactive groups (up to 10 people) are held over Zoom.
