Completion of the certificate in care coordination provides individuals with a foundation to synthesize available evidence to lead interprofessional teams to design, deliver and evaluate care coodination practices that are patient-centered; improve health, safety and cost outcomes; and enhance quality and access to care across diverse populations. This program supports preparation for national certification as a case manager through the American Nurses Credentialing Center or through the Case Management Society of America.
The Plan of Study is the required curriculum to complete the program.
