HOME-MIND (HOme-based Monitoring & Enabled Medication Independence with New Digital tools)
Project Description:
This study evaluates HOME-MIND, a digital health intervention designed to support medication self-management among adults aged 65 and older who live alone with subjective cognitive decline or mild cognitive impairment and take multiple medications. The intervention includes daily medication safety self-monitoring, personalized medication education, and tools to support communication with healthcare providers. Researchers will examine its effects on medication self-management, medication adherence, and medication burden.
