Traditional Prelicensure Clinical Nursing Application for Advancement

The Traditional BSN (TBSN) is intended for students working on their first bachelor’s degree. Applicants with a regionally accredited bachelor’s degree are expected to apply for the Accelerated BSN or the MS Nursing (Entry to Nursing Practice).

Advancement Pathways*

Traditional BSN cohorts are offered in summer and fall, and may be offered at the following campuses: Downtown Phoenix, West, or Polytechnic campus. Additionally, there is an Accelerated BSN cohort offered at ASU at Lake Havasu starting in the fall. Students must meet additional requirements to be considered for the Accelerated BSN cohort at Lake Havasu.

Indicate your cohort preference by ranking the options below. You must rank all five options. Each selection below must be different. You cannot select the same choice multiple times. Placement in your preferred cohort is not guaranteed.

Edson College first bachelor’s degree-seeking students may also apply to the Accelerated BSN cohort for the Lake Havasu campus. There are additional requirements and space is not guaranteed. If you would like to be considered for the Accelerated BSN program, you must submit an additional Accelerated BSN Advancement Application with the Gold Form.

Admissions and Transfer Coursework

If you have remaining prerequisites that you will be in progress, list them in the space provided. Include one course per line with course code, institution, credit hours, and end date. If all prerequisites are completed, please write “Complete”. Refer to the summer/fall 2023 Advancement Guide for prerequisite course deadlines.

Non-Academic Items Required for Advancement

Test of Essential Academic Skills (TEAS)
Applicants are allowed one attempt at the TEAS per application cycle unless a failing score below 58.7% is earned. If a student earns less than 58.7% one retest is permitted, but the student must wait for a minimum of 30 days between tests, and the second attempt must be on or before February 1, 2023.
I have contacted ATI Testing to request that the official transcript be sent to ASU; and I have confirmed that the official transcript has been received by the Edson College of Nursing and Health Innovation before the application deadline.

Health and Safety Requirements

Applicants must create a CastleBranch account by the advancement deadline of February 1, 2023. All items must be completed by March 1, 2023, except for the drug and alcohol screening, criminal background check, and statement of health clearance. Applicants who do not meet these stated deadlines will be considered late applicants and will only be placed if requirements are met and space is available. 

Students identified for advancement or alternate status must complete the remaining health and safety requirements (drug and alcohol screening, criminal background check, and statement of health clearance) by April 2, 2023, to maintain eligibility. 

Steps to Set Up CastleBranch Account and Submit Health and Safety Documents
      1.    Visit CastleBranch’s website: https://mycb.castlebranch.com
      2.    Upper-right hand corner, enter the Package Code and select Go.
      3.    Package Code: AH91im
      4.    Review the information on the next page.
      5.    Pay the $38 fee for the account/Medical Document Manager.
      6.    Once you click continue, you will have access to submit documentation to your tracker. You can use the FAQ video library for assistance with common inquiries.
      7.    Check your portal until CastleBranch accepts each documentation and marks it complete.

Influenza (Flu) Vaccination
Administered within the current flu season (vaccine must have been given after August 1, 2023
CPR American Heart Association BLS Provider

To authorize the college to release health, immunization, and other information to clinical placement agencies. (Available after advancement or alternate status)

Statement of Disclosure
Acknowledge that they may be at risk of known or unknown exposure to devices, equipment, or treatments that may be potentially harmful. (Available after advancement or alternate status)

COVID-19 Vaccination Students should complete the full series (1-2 doses depending on the manufacturer) of an authorized COVID-19 vaccine.

Health Insurance Coverage
Students can purchase private health insurance or enroll in ASU student health insurance. Bridge Discount Plan is not eligible.
 

Renewal:
If CPR, Tdap, IVP Clearance Card, or TB renewal date falls within January 1 - April 30, students must renew prior to December 20th.
If CPR, Tdap, IVP Clearance Card, or TB renewal date falls within May 1 - July 31, students must renew prior to April 20th.
If CPR, Tdap, IVP Clearance Card, or TB renewal date falls within August 1 - December 31, students must renew prior to July 20th.
